Current HMO Members: If you are not making any enrollment changes and only require Primary Care Physician changes, please contact the carrier directly to reflect the change by calling the member services phone number on the back of your ID Card.
Open Enrollment is the one opportunity each year when you can make changes to your Benefits and insurance selections for any reason.
Making Benefit Changes During the Plan Year
If you have a “qualifying life event,” you may make changes to certain benefits if you apply for the change and provide supporting documentation to Human Resources within 30 days of the event. Proof of life events is subject to approval. Documentation is required to be submitted timely. Changes are effective prospectively unless the event is for birth, adoption, or placement for adoption.
Eligibility
You are eligible for benefits if you are:
All full-time staff who work at least 30 or more hours per week and have satisfied their waiting period of first of the month following date of hire.
