If you have a “qualifying life event,” you may make changes to certain benefits if you apply for the change and provide supporting documentation to Human Resources within 30 days of the event. Proof of life events is subject to approval. Documentation is required to be submitted timely. Changes are effective prospectively unless the event is for birth, adoption, or placement for adoption.
Making Benefit Changes During the Plan Year
Eligibility
You are eligible for benefits if you have satisfied your waiting period of first of the month following the date of hire and meet the required hours as follows:
Medical - Dental - Vision
All full-time employees who work at least 30 or more hours per week
Basic Voluntary Life/AD&D - Voluntary Accident - Voluntary Critical Illness
All full-time employees who work 20 or more hours per week
Short-Term Disability
All full-time employees working 35 hour or more
Whole Life Insurance
All employees who work 20 or more hours per week. You can purchase this policy to cover your entire life, as long as you keep paying premiums. Premiums remain constant throughout the policy, and the company invest a portion of your premium that becomes the cash value. These are more expensive than term policies in the early years, but they even out because the premium does not increase.
